I had a metal JR arm(star washer with centre bolt and clamp bolt) pop off an aileron servo in the early flights of my Sbach. I added a little blue loctite to everything after that and no issue in the subsequent 4 years.

Tales of servo woe might be indicative of lack of maintenance (I plead guilty too). Star washers flatten and aluminium and plastic squashes so it really should be a compulsory check that all screws are tight at the beginning of every flying session. Loctite (blue) may only mask the problem as a loose arm will soon strip its splines even if the screw doesn't come loose any more.

Not wishing to hijack an informative thread about Mythos Throws ;) I can own up to a tiny drop of blue on the Beasts rudder servo after the ally arm fell off. Never had an issue with hd plastic arms though, probably a tighter fit on the splines.
